Non-OHIP Services
Many patients are surprised to discover that not all of their medical needs are covered under the Ontario Health Insurance Plan (OHIP), and that they must pay their provider directly for certain uninsured services
Examples of some common uninsured services include:
A service that is solely for the purpose of altering or restoring appearance.
Providing a prescription to an insured person if the person, person’s personal representative or pharmacy requests the prescription and no concomitant insured service is provided.
Completion of third party reports and forms such as insurance letters.
Our clinic has partnered with the Ontario Medical Association’s (OMA) recommended partner - PatientServ, to provide payment options for patients including paying a la carte for services or paying for an annual plan exclusive for family medicine patients which includes many of the non-OHIP services generally requested. This may be considered a tax deducible medical expense.

Policies and Procedures
No-Show Policy
At Unify Health, we strive to provide timely and efficient care to all our patients. To ensure that we can maintain this level of service, we have implemented a policy regarding missed appointments. If a patient fails to provide at least 48 hours notice of cancellation, except in cases of emergency, a fee will be charged. This fee must be settled before any further assessments can be scheduled. The amount of the fee varies depending on the type of appointment missed. This policy helps us manage our schedule and resources effectively, ensuring that all patients have access to the care they need when they need it
IV Iron
At Unify Health, we provide a comprehensive set of documentation for our IV iron infusion services, including consent forms, informational packages, and post-infusion instructions. Please note there is a charge of $150-$200 per infusion. Here is the information you'll need.

Respect and Safety Policy
At Unify Health, we are committed to providing a safe and respectful environment for all patients, staff, and visitors. We have a zero-tolerance policy for any form of aggressive, verbal, or physical behavior that intimidates or threatens our staff or other patients both in person and over the phone or email. Such behavior is unacceptable and will not be tolerated under any circumstances. Patients exhibiting such behavior may be asked to leave the premises immediately and may be discharged from the practice. Our goal is to ensure that everyone at Unify Health feels safe and respected, and we appreciate your cooperation in maintaining a positive and supportive environment.
Unify Health Clinic Privacy Notice
The information below comprises a summary of the privacy and information policies within the Unify Health Clinic and all other practitioners and their agents working in the Unify Health Clinic. A shared Electronic Medical Record (EMR) is used by practitioners and their agents working at the Unify Health Clinic. Only those practitioners or their agents involved in the patient’s care are to access a patient’s medical record.
Collection of Personal Health Information
We collect personal health information about you directly from you or from the person acting on your behalf. The personal health information that we collect may include, for example, your name, date of birth, address, health history, records of your visits to Unify Health Clinic and the care that you received during those visits. Occasionally, we collect personal health information about you from other sources if we have obtained you consent to do so or if the law permits.
Uses and Disclosures of Personal Health Information
We use and disclose your personal health information to:
• Treat and care for you
• Get payment for your treatment and care (from OHIP, WSIB, your private insurer or others)
• Plan, administer and manage our internal operations
• Conduct risk management and quality improvement activities
• Teach/Education
• Conduct research
• Compile statistics
• Comply with legal and regulatory requirements
• Fulfil other purposes permitted or required by law
Your Choices
You may access and correct your personal health records, or withdraw your consent for some of the above uses and disclosures by contacting us (subject to legal exceptions).
Important Information
• We take steps to protect your personal health information from theft, loss and unauthorized access, copying, modification, use, disclosure and disposal.
• We conduct audits and complete investigations to monitor and manage our privacy compliance.
• We take steps to ensure that everyone who performs services for us protect your privacy and only use your personal health information for the purposes you have
